A 70-year-old man with prior left parietal meningioma resection and gamma knife treatment for ventricular recurrence underwent partial hepatectomy of segments 6 and 8 for multiple hepatic masses with a nondiagnostic percutaneous biopsy.

Immunohistochemistry positive for vimentin, EMA, and somatostatin receptor 2 confirmed metastatic meningioma.

This has no valve relevance, and we will not pretend otherwise.

It appears in our feed through source overlap, not clinical overlap.

Source Abstract

INTRODUCTION: Meningioma is one of the most common central nervous system tumors; however, metastatic lesions are extremely rare. Particularly, hepatic metastasis from meningioma is uncommon, and its diagnosis and optimal management remain undefined. CASE PRESENTATION: A 70-year-old man with a history of left parietal meningioma resection in 2011 and gamma knife therapy for recurrence in the left lateral ventricle in June 2025. A PET-CT scan performed for cancer screening in June 2025 revealed abnormal fluorodeoxyglucose uptake in segment 8 of the liver, with a maximum standardized uptake value of 6.2. CT, and MRI revealed multiple hepatic masses. All tumor markers were within normal ranges. A percutaneous liver biopsy indicated a spindle cell tumor; however, the diagnosis remained inconclusive. For diagnostic and therapeutic purposes, partial hepatectomy of segments 6 and 8 and cholecystectomy were performed. Histopathological examination revealed proliferating spindle-shaped tumor cells arranged in fascicles. Immunohistochemical staining was positive for vimentin, epithelial membrane antigen, and somatostatin receptor 2, confirming the diagnosis of hepatic metastasis from meningioma. The postoperative course was uneventful, and the patient was discharged in good condition. CONCLUSIONS: Hepatic metastasis from meningioma is extremely rare; however, it should be considered in the differential diagnosis of hepatic tumors in patients with a history of meningioma. Surgical resection is valuable in establishing a definitive diagnosis and achieving local disease control.
