The Valve Wire

Monday, October 12, 2026

Executive Summary

Canada's national discharge data now shows TAVI volumes running at roughly twice SAVR volumes, with transcatheter procedures climbing from 19.0 to 136.9 per million while surgical replacement fell from 104.4 to 66.5 per million between 2012 and 2024, and the steepest crossover occurring in patients aged 60 to 79 (CJC Open). That is the demographic where ACC/AHA 2020 still calls SAVR Class I below 65 and where ESC 2025 holds the SAVR line to 70, so practice has outrun both documents. On the same day, JenaValve issued a Class II recall on 362 Trilogy delivery catheters for sleeve liner separation, a reminder that the device pipeline expanding fastest (TAVI for aortic regurgitation) has the thinnest surveillance behind it. Meanwhile a pooled analysis of 2,771 patients found no mortality separation between Myval and Sapien 3 at one year despite better Myval hemodynamics (Acta Cardiologica), which complicates the case that gradient advantages translate into survival.

  • Skirted self-expanding valves tripled the odds of subclinical leaflet thrombosis after TAVR (adjusted OR 2.999) in a 311-patient multicenter CT cohort (Rev Cardiovasc Med).
  • Canadian societies issued a joint practice update on transcatheter mitral and tricuspid intervention, centered on referral timing and heart team structure (Can J Cardiol).
  • Valve-in-valve inside internally stented surgical valves: self-expanding devices beat balloon-expandable on 1-year gradient (11.7 vs 22.7 mm Hg) with no survival difference (CJC Open).
  • Post-TAVR anticoagulation guidance diverges across ESC/EACTS 2025, ACC/AHA 2020, and NICE because no trial has enrolled new-onset post-procedural AF (Perfusion).
  • Edwards closed at $86.39, up 1.38%, ahead of November 4 earnings and following FDA clearance of its AUTUS valve (Simply Wall Street).

What to watch: EACTS late-breaking sessions run through October 11, and the surgical response to the ESC 2025 age-70 threshold is the session to track.

Aortic Valve (TAVR/TAVI)

Hemodynamic superiority is not a survival argument, and today's Myval data makes that explicit. The updated meta-analysis of five studies and 2,771 patients found Myval lowered mean gradient by 2.37 mm Hg and increased effective orifice area by 0.26 cm2 versus Sapien 3 at 30 days, with the benefit sustained at one year, yet 30-day mortality (RR 1.05) and one-year mortality (RR 0.93) were flat, as were stroke, bleeding, and pacemaker rates (Acta Cardiologica). Five studies is a thin pool and most are non-randomized registries; the two RCTs contributing one-year data are the only hard evidence here. [NOTABLE] Separately, skirted self-expanding prostheses carried an adjusted OR of 2.999 for subclinical leaflet thrombosis in 311 patients imaged by 4D-CT within six months, with HALT in 29.3% and reduced leaflet motion in 14.5% (Rev Cardiovasc Med, retrospective, two centers, no surgical comparator).

Surgical counterpoint: no equivalent skirt-related thrombosis signal exists for stented surgical bioprostheses, and ACC/AHA 2020 still places SAVR as Class I below 65 on durability grounds. The leaflet thrombosis literature is precisely the mechanism that could erode TAVI durability at 7 to 10 years, and it is device-design dependent in a way surgical valves are not.

Mitral Valve (MitraClip, PASCAL, TMVR)

Canada just put its name on a transcatheter mitral pathway, and the operative word in the document is referral. The joint CCS/CSCS/CAIC clinical practice update covers transcatheter edge-to-edge repair and transcatheter mitral replacement, with the stated central theme being timely referral and structured multidisciplinary assessment to avoid missed intervention windows (Can J Cardiol). This is a consensus document, not new trial data, and it should be read as such.

Where it sits against guidelines matters. ESC 2025 elevated TEER for ventricular secondary MR to Class I LOE A on the strength of COAPT 5-year data, RESHAPE-HF2, and pooled analysis; ACC/AHA 2020 holds at Class IIa. For primary MR, both documents still subordinate TEER to surgical repair, Class IIa and only for high surgical risk with suitable anatomy, and both reserve Class I for surgical repair when a durable result is expected. A practice update that emphasizes earlier referral cuts both ways: earlier referral should route more degenerative MR to repair-capable surgeons, not fewer. The surgical repair literature remains the benchmark here, with durable freedom from reoperation at 15 to 20 years that no transcatheter device has approached.

Tricuspid Valve (TriClip, TTVR)

The tricuspid content today is procedural, not comparative. The same Canadian practice update folds transcatheter tricuspid edge-to-edge repair and replacement into its evaluation framework, again stressing that patients arrive too late (Can J Cardiol). A separate case report describes percutaneous aspiration of native tricuspid valve endocarditis from a hemodialysis catheter using the Flash 2.0 system (CJC Open, single case, no abstract data).

Guideline context: ESC 2025 rates transcatheter tricuspid treatment Class IIa LOE A for high-risk symptomatic severe TR without severe RV dysfunction or pre-capillary pulmonary hypertension, built on TRILUMINATE Pivotal, Tri.Fr, and TRISCEND II. ACC/AHA 2020 contains no recommendation at all, because the trials had not read out. ESC also upgraded surgical TV repair for symptomatic severe primary TR to Class I. That upgrade gets less attention than the transcatheter one and deserves more: the composite endpoints driving TRILUMINATE and Tri.Fr were carried by quality-of-life instruments in unblinded trials, not mortality. Percutaneous debulking of vegetations in endocarditis remains outside any guideline and should not be read as an alternative to valve surgery in patients who can tolerate it.

Surgical vs. Transcatheter Comparisons

The Canadian population data is the day's most honest picture of where the field actually went. Across 2012 to 2024, AS hospitalizations rose from 259.8 to 362.4 per million, SAVR fell to 66.5 per million, and TAVI reached 136.9 per million, with the crossover concentrated in patients 60 to 79 years old (CJC Open). Administrative data cannot adjudicate appropriateness, and the authors flag persistent sex and geographic disparities: TAVI rates were higher in men, AS hospitalizations higher in rural regions. Still, the 60-to-79 shift runs directly through the ACC/AHA 2020 shared-decision band (65 to 80) and through the ESC 2025 SAVR-preferred zone (under 70). Volume moved before the evidence for that age stratum did.

The downstream cost is visible in two surgical commentaries published today on managing the small aortic annulus in an era of rising TAVR explant (J Vis Surg) and on the Commando procedure for invasive endocarditis of a transcatheter valve (J Vis Surg). Neither carries outcome data. Both describe the operations that follow when index-procedure planning ignores lifetime management, which ESC 2025 named as a formal consideration and ACC/AHA 2020 did not. THV explant mortality sits at 12% to 17%.

Device & Technology

Valve-in-valve device choice inside internally stented surgical bioprostheses now has propensity-matched data. Among 113 patients undergoing ViV-TAVI in Mitroflow or Trifecta valves, 37 matched pairs showed equivalent 30-day clinical outcomes and no difference in the 2-year composite of death, HF hospitalization, and coronary obstruction, but supra-annular self-expanding valves delivered a markedly lower 1-year mean gradient than balloon-expandable valves (11.7 vs 22.7 mm Hg, P<0.001) (CJC Open, retrospective, single-center, small matched cohort). A 22.7 mm Hg mean gradient at one year after a ViV procedure is not an acceptable hemodynamic result in a patient with life expectancy beyond a decade, and it is an argument for surgical explant and root enlargement in the right candidate.

Four cases of Navitor implantation into protruding LVOT calcific nodules achieved no more than mild-to-moderate paravalvular regurgitation (JACC Case Reports, N=4, explicitly hypothesis-generating). LVOT calcification is a guideline-listed factor favoring SAVR in both documents. A scoping review of 11 machine learning studies covering 101,030 patients found Random Forest predicted post-TAVR mortality with AUROC 0.66 to 0.97, a range wide enough to make the point that none of these models is ready for the clinic (CJC Open).

Regulatory & Policy

[NOTABLE] JenaValve recalled 362 Trilogy delivery catheters across the 23-mm (111 units), 25-mm (108 units), and 27-mm (143 units) sizes after reports that sleeve liner separation could make deployment difficult or impossible. The FDA classified it Class II, meaning temporary or reversible harm (Cardiovascular Business). Context raises the stakes: Trilogy became the first THV approved in the US specifically for aortic regurgitation in March, and CMS is weighing an updated national coverage determination to extend Medicare TAVR coverage to AR. No final decision yet. ESC 2025 rates TAVI for AR Class IIb and restricts it to inoperable patients with suitable anatomy; ACC/AHA 2020 has no AR TAVI recommendation at all. AR remains a surgical disease in both documents, with valve-sparing root replacement Class I in young patients under ESC 2025. A delivery system failure in the one indication where transcatheter therapy has the least guideline support is a bad combination, and operators should quarantine affected lots immediately.

Separately, guideline divergence on post-bioprosthetic AVR anticoagulation reflects an absent evidence base rather than genuine equipoise: no trial has enrolled patients with new-onset post-procedural AF or stratified by AF subtype (Perfusion).

Industry & Market

Siemens Healthineers is exiting general imaging ultrasound to concentrate on cardiovascular care (Cardiovascular Business). Structural heart procedural growth depends on imaging capacity as much as on valve supply, and the Canadian volume data explains why a large imaging vendor would make that bet. A separate market forecast projects transcatheter valve delivery device growth through 2035 on TAVR expansion and aging demographics (IndexBox). The JenaValve recall is a reminder that the delivery system, not the valve, is where these projections carry execution risk. Oklahoma Heart Institute's TAVR program earned a 3-star STS/ACC TVT Registry rating (KOKI FOX 23).

Financial Analysis

Edwards is the clean read-through from today's clinical news. The stock closed at $86.39, up 10.59% over six months, trading at 51.4x trailing and 25.6x forward earnings, with 26 analysts averaging a $101.96 target. FDA approval of the AUTUS valve is the catalyst being priced, and the November 4 print (consensus $0.74 EPS on $1.68B revenue) will show whether TAVI volume growth of the kind Canada documented is translating into Edwards units or being split across Myval, Navitor, and the self-expanding field. The Myval meta-analysis matters commercially: equivalent mortality with better gradients at a lower price point is a procurement argument, and Meril is private, so the pressure shows up in Edwards margins rather than in a competitor's stock.

Boston Scientific is the outlier, down 30.61% over six months and sitting at its 52-week low of $41.39 despite a 2.60% bounce today. Analyst targets span $44 to $94, which is not a consensus. None of today's sources explains the derating; October 28 earnings will. Medtronic gained 3.90% on heavy volume (36.6M shares) and Abbott 2.23%, both ahead of prints that include structural heart segment detail.

The pure-play structural heart name. AUTUS approval and the TRISCEND II franchise are the growth story; the counterweight is balloon-expandable share erosion from Myval, where today's pooled data showed equivalent one-year survival and better gradients.

Medtronic (MDT)

MDT 6-Month Chart
  • Close $91.93, up $3.45 (+3.90%); 6-month change +$5.43 (+6.27%)
  • Market cap $117.6B; P/E trailing 22.59, forward 14.33; beta 0.57
  • 52-week range $73.31 to $106.33; volume 36.6M

  • Next earnings November 17, 2026 (EPS est $1.33, revenue est $9.48B)

The strongest single-day move in the group on outsized volume. Evolut supra-annular design benefits directly from the ViV hemodynamic finding published today, where self-expanding valves halved the 1-year gradient inside internally stented surgical bioprostheses.

Abbott (ABT)

ABT 6-Month Chart
  • Close $101.82, up $2.22 (+2.23%); 6-month change +$2.47 (+2.48%)
  • Market cap $177.4B; P/E trailing 32.85, forward 16.79; beta 0.58
  • 52-week range $81.97 to $133.34; volume 7.01M
  • Analyst target $120.72 (range $103 to $135, 26 analysts); consensus strong buy
  • Next earnings October 21, 2026 (EPS est $1.42, revenue est $13.00B)

MitraClip and TriClip sit inside a diversified base, which mutes valve-specific signal. The ESC 2025 Class I for TEER in ventricular secondary MR is the structural tailwind; the open question is whether US reimbursement follows before ACC/AHA updates.

Boston Scientific (BSX)

BSX 6-Month Chart
  • Close $43.84, up $1.11 (+2.60%); 6-month change -$19.34 (-30.61%)
  • Market cap $63.5B; P/E trailing 17.75, forward 12.97; beta 0.57
  • 52-week range $41.39 to $105.65, currently at the low
  • Analyst target $59.72 (range $44 to $94, 29 analysts); consensus buy
  • Next earnings October 28, 2026 (EPS est $0.75, revenue est $5.11B)

A 30% six-month drawdown with a $50 spread in analyst targets signals genuine disagreement about the forward book. ACURATE neo2 withdrawal from the US market removed the structural heart aortic story; October 28 is the catalyst.

Anteris Technologies (AVR.AX)

AVR.AX 6-Month Chart
  • Close $9.59, up $0.04 (+0.42%); 6-month change +$1.79 (+22.95%)
  • Market cap $0.9B; forward P/E -4.43 (pre-revenue); volume 5,592
  • 52-week range $5.20 to $15.47
  • Single analyst target $13.00; no consensus recommendation

DurAVR's single-piece ADAPT tissue design is a durability bet, which is the right thesis given where the TAVI evidence is weakest. Thin volume makes the price unreliable as a signal.

JenaValve Technology, J Valve Technology, and Meril Life Sciences are private; no public stock data available. The group's one-day strength across four of five names contrasts with Boston Scientific's six-month collapse, and the dispersion tells the real story: structural heart is no longer a rising tide. Device-specific evidence, hemodynamic performance, and now delivery-system reliability separate winners from losers.

Clinical Trial Updates

POPular PET TAVI is enrolling toward the question that determines whether TAVI holds up past a decade. The international cross-sectional study will image 180 patients at five years post-TAVI with 4D-CT for leaflet thrombosis, neosinus volume, prosthesis deformation, expansion, and commissural alignment, plus 18F-sodium fluoride PET to quantify leaflet microcalcification activity as an early marker of structural valve deterioration (J Nucl Cardiol). Enrollment is stratified by valve size and type to compare balloon-expandable intra-annular against self-expanding supra-annular designs; 159 patients enrolled as of September 2026, completion expected December 2026. Cross-sectional design and core-lab adjudication are strengths; the absence of a surgical bioprosthesis comparator arm is the limitation. Taken alongside today's skirt-associated thrombosis signal, mechanism is finally being studied at the same pace as adoption.

Social & Conference Highlights

The EACTS Annual Meeting is underway through October 11, and no late-breaking valve trial from the sessions reached today's indexed feed. The surgical commentaries that did land, on small annulus management amid rising TAVR explant and on Commando reconstruction for transcatheter valve endocarditis, read as the operative sequelae of a decade of volume shift. That is the conversation EACTS should be having in response to the ESC 2025 age-70 threshold.

Watch three things over the next fortnight: whether CMS moves on the TAVR-for-AR coverage determination now that the sole approved device has a Class II delivery catheter recall against it, whether Boston Scientific's October 28 print explains a 30% derating, and whether EACTS produces a surgical rebuttal to an age threshold that Canadian population data suggests the field already crossed without waiting for the evidence.
