Executive Summary

Residual mitral regurgitation after TEER independently predicts five-year mortality, with 67.7% mortality in patients left with MitraScore ≥4 versus 55.1% in those achieving mild or less residual MR — a 12.6-point gap that hardens the case for intraprocedural quantitative assessment and, in surgical-eligible patients, revives the durability argument for repair (MITRA-PRO 5-year, JACC: Cardiovascular Interventions). The multicenter SUBLIME study validated the LIRA sizing method for bicuspid TAVR across 215 patients (93% device success, 1.9% stroke), and HighLife closed $90M+ to push its TMVR platform — both signals that the transcatheter field is trying to solve the anatomies that surgery still owns, which tightens rather than settles the SAVR-vs-TAVI question at the margins.

  • MITRA-PRO 5-year data: the relationship between residual MR and mortality is near-linear, prognostic beyond 30 days in both DMR and FMR cohorts (JACC Interv).
  • SUBLIME multicenter registry: LIRA method drove prosthesis downsizing in 74.4% of BAV TAVR cases, with 4.6% ≥moderate PVL and 10.7% new PPM (JACC Interv).
  • HighLife raised >$90M for its dual-component transcatheter mitral valve system, extending TMVR's runway despite persistent access and durability questions (MassDevice).
  • Piedmont Atlanta added Carlos Sanchez to expand its EVOQUE TTVR and ICE program, another sign that transcatheter tricuspid replacement is scaling faster than long-term data (ACCESS Newswire).
  • Rigshospitalet launched a 1,986-patient RCT on peri-procedural antibiotic prophylaxis for TAVI, targeting Enterococcus faecalis coverage — the first adequately powered trial to address post-TAVI IE prevention (NCT07442357).

What to watch: Edwards Lifesciences reports Q3 earnings on October 29, the first read on TAVR and TEER volume trends since the EARLY TAVR asymptomatic data began influencing referral patterns.

Aortic Valve (TAVR/TAVI)

Bicuspid TAVR sizing took a step toward standardization. The SUBLIME multicenter study (N=215, retrospective) reported that the LIRA (level of implantation at the raphe) method achieved 93% device success at 30 days, 4.6% ≥moderate paravalvular leak, 10.7% new pacemaker, and 1.9% stroke, with 5.7% all-cause mortality at one year. Prosthesis sizing agreement across centers was excellent (κ=0.85). The method drove downsizing in three-quarters of cases — a meaningful shift given that oversizing has historically driven both PVL and annular injury in bicuspid anatomy. The accompanying editorial asks the right question: reproducibility is not the same as superiority. There is no randomized comparison to standard annular sizing, and the ESC 2025 guidelines hold TAVI for bicuspid AS at increased surgical risk at Class IIb — a rating grounded in NOTION-2's numerically worse bicuspid TAVI signal (HR 3.8, P=0.07) and the near-total exclusion of BAV patients from pivotal low-risk RCTs. ACC/AHA 2020 similarly rates TAVI for BAV at Class IIb. SUBLIME does not change that calculus for young bicuspid patients, where both ACC/AHA and ESC continue to favor SAVR, particularly when concomitant aortopathy is present. Separately, an EHJ editorial reframes post-TAVI FFR as a continuum between epicardial and microvascular physiology — residual gradient physiology after TAVR is more nuanced than the number on the wire suggests.

Mitral Valve (MitraClip, PASCAL, TMVR)

[NOTABLE] Residual MR after M-TEER kills. The MITRA-PRO 5-year follow-up (N=1,491) shows a near-linear relationship between intraprocedural MitraScore and five-year mortality: 55.1% in patients with MitraScore ≤3 vs 67.7% with ≥4. The gap was largest in degenerative MR (53.1% vs 73.2%), where surgery is the guideline-preferred alternative — ACC/AHA 2020 (Class IIa) and ESC 2025 both keep TEER subordinate to surgical repair for primary MR when repair is feasible, and the DMR mortality gap in MITRA-PRO makes that hierarchy concrete. Predictors were time-dependent: early death was driven by comorbidity and procedural factors, but residual MR became the dominant prognostic signal beyond 30 days. This is a European multicenter registry, not randomized, and MitraScore incorporates operator-graded parameters. Even so, the effect size and its persistence out to five years reframe the "MR reduction to ≤2+" success criterion as inadequate for degenerative disease. The accompanying editorial makes the case directly: grade alone is inadequate; quantitative multimodal assessment should guide intraprocedural strategy. Separately, intraprocedural left atrial pressure monitoring during M-TEER published today adds another physiologic tool to that assessment set.

Tricuspid Valve (TriClip, TTVR)

Piedmont Atlanta's addition of Carlos Sanchez to its EVOQUE TTVR and ICE program (ACCESS Newswire) is a small news item with a larger implication: transcatheter tricuspid replacement is scaling into community and regional programs faster than long-term outcome data can support. ESC 2025 gave transcatheter TV treatment Class IIa (LOE A) for high-risk symptomatic severe TR without severe RV dysfunction, based on TRILUMINATE Pivotal, Tri.Fr, and TRISCEND II — all of which showed QoL and HF hospitalization benefits but no definitive hard-outcome mortality signal. ACC/AHA 2020 does not address transcatheter TR therapy at all, leaving the widest guideline gap in structural heart. TRISCEND II showed TTVR delivers superior symptom relief compared to repair but with meaningfully higher bleeding and pacemaker rates; the STS/ACC TVT Registry real-world data reported 3.1% 30-day mortality and 15.9% new pacemaker implantation. Surgical isolated TV replacement carries 8–20% mortality in historical series, though earlier referral is improving that figure. The scaling of TTVR programs into more centers should not obscure the fact that no randomized comparison to surgery exists, and outcome data beyond two years are absent for most devices.

Surgical vs. Transcatheter Comparisons

No head-to-head studies today. The day's findings sit uneasily against surgical comparators in three ways: MITRA-PRO's residual MR mortality gradient argues for surgical repair whenever a durable low-residual result is achievable in DMR; SUBLIME's bicuspid TAVR results are procedurally reassuring but do not touch the durability question that keeps ACC/AHA and ESC at SAVR-preferred for younger BAV patients; and Piedmont's TTVR expansion outpaces any randomized surgical comparator, of which there is none.

Device & Technology

A JACC Interv case series reports incomplete endothelialization of a fifth-generation balloon-expandable THV skirt associated with hemolysis — a reminder that iterative valve design changes carry unanticipated late-signal risks. Transfemoral TAVR with a 35-mm Myval valve in an extremely large annulus using intentional undersizing extends the anatomic envelope for balloon-expandable platforms, consistent with the LANDMARK trial's non-inferiority signal at one year. Hybrid TAV-in-TAV with low implantation and bilateral snorkel stenting illustrates the lifetime-management complexity the ESC 2025 guidelines flagged: coronary access planning at the index procedure is not optional. A structured pre-procedural risk mitigation case in a 97-year-old with 1,178 mm³ calcium volume used pre-emptive ECMO backup and managed intraprocedural collapse — a template for the ultra-high-risk end of the TAVI population.

Industry & Market

HighLife raised more than $90M to advance its transatrial TMVR system with a dual-component anchoring design. Capital is still flowing into TMVR despite a decade of clinical setbacks — Abbott's Tendyne, Medtronic's Intrepid, and Edwards's SAPIEN M3 have all faced access, LVOT obstruction, or durability challenges. HighLife's differentiator is a subannular ring that seats the valve, potentially reducing LVOT obstruction risk. Whether that translates into a viable clinical program remains to be shown; ESC 2025 gives TMVI for degenerative MS with MAC only a Class IIb recommendation, and no dedicated recommendation for primary or secondary MR TMVR exists outside of trials.

Financial Analysis

Structural heart stocks traded softer with the broader medtech tape. Edwards (EW) fell 0.36% but remains up 8.8% over six months, holding on the strength of TAVR volume expectations post-EARLY TAVR and TEER growth. Boston Scientific (BSX) is down 31% over six months, the standout underperformer in the space, weighed by product cycle timing and competitive concerns in electrophysiology. Medtronic (MDT) fell 2.34% on the day. HighLife's $90M raise underscores that private capital still sees TMVR as a category worth funding despite the graveyard of prior programs — the market signal here is a bet on anatomic differentiation, not on the current clinical data.

  • Analyst target: $100.96 (buy, 26 analysts)
  • Next earnings: Oct 29, 2026 (EPS est $0.74, Rev est $1.68B)
  • Commentary: The Q3 print will be the first clean read on whether EARLY TAVR is translating into asymptomatic AS referral volume. TEER (PASCAL) and TTVR (EVOQUE) commercial trajectories matter equally — Piedmont's EVOQUE program expansion supports the volume story. Rams partnership extension and 20-year Singapore anniversary are brand plays, not fundamentals.

Medtronic (MDT)

MDT 6-Month Chart
  • Close: $87.40 (-2.34%); 6-month: +3.71%
  • Market cap: $111.8B; P/E trailing 21.5, forward 13.6; beta 0.57; 52-week: $73.31–$106.33
  • Analyst target: $104.83 (buy, 24 analysts)
  • Next earnings: Nov 17, 2026 (EPS est $1.33, Rev est $9.48B)
  • Commentary: Evolut has held share in the TAVR duopoly but faces intensifying competition from Myval (LANDMARK non-inferiority) at the balloon-expandable end. Intrepid TMVR remains in slow development mode.

Abbott (ABT)

ABT 6-Month Chart
  • Close: $99.78 (-1.19%); 6-month: -0.74%
  • Market cap: $173.8B; P/E trailing 32.3, forward 16.5; beta 0.59; 52-week: $81.97–$135.45
  • Analyst target: $120.26 (buy, 25 analysts)
  • Next earnings: Oct 14, 2026 (EPS est $1.42, Rev est $12.99B)
  • Commentary: MitraClip franchise faces the MITRA-PRO 5-year residual MR mortality signal — a data point that could sharpen intraprocedural quality metrics and, indirectly, referral patterns. TriClip commercial ramp continues against the EVOQUE TTVR alternative.

Boston Scientific (BSX)

BSX 6-Month Chart
  • Close: $43.42 (-1.55%); 6-month: -30.99%
  • Market cap: $62.9B; P/E trailing 17.6, forward 12.7; beta 0.57; 52-week: $42.20–$105.65
  • Analyst target: $61.00 (buy, 29 analysts)
  • Next earnings: Oct 28, 2026 (EPS est $0.77, Rev est $5.15B)
  • Commentary: The 31% six-month drawdown makes BSX the sector's laggard. ACURATE neo2 discontinuation last year left BSX without a TAVR platform, isolating structural heart exposure to niche products.

Anteris Technologies (AVR.AX)

AVR.AX 6-Month Chart
  • Close: A$10.97 (+1.95%); 6-month: +42.47%
  • Market cap: A$1.1B; forward P/E -5.06; 52-week: A$5.20–A$15.47
  • Analyst target: A$13.00 (1 analyst)
  • Commentary: DurAVR THV early-feasibility momentum drove the six-month rally; commercial pathway remains long.

Sector outlook: Consolidation of the TAVR duopoly is stable; the growth frontiers are TEER quality (MITRA-PRO), TTVR commercial scaling (EVOQUE), and TMVR feasibility (HighLife). The financing environment for privately held structural heart companies remains constructive.

Clinical Trial Updates

Aortic

  • NCT07442357 — Per-procedural Antibiotic Prophylaxis in TAVI: Recruiting, N=1,986, sponsor Rigshospitalet Denmark. Comparing beta-lactam with Enterococcus faecalis spectrum vs cephalosporin. First adequately powered RCT to address post-TAVI infective endocarditis prevention — a meaningful gap given the rising IE incidence in the TAVI population.

Preprint Highlights

Nothing structural heart-relevant in today's preprint feed; the single bioRxiv item concerned multiplexed hormone immunoassays for women's health.

Forward look: The next inflection is Edwards Q3 earnings (Oct 29) and Abbott Q3 (Oct 14), which together will reveal whether the EARLY TAVR and MITRA-PRO signals are moving referral and quality metrics in the real world.
